I understand that as a parent/guardian of the listed skater(s), I am a Special Member of the Beausejour Skating Club (BSC)
and will equitably support the BSC. Further, I agree and waive all rights to any claim or action against the BSC, the members
if the BSC Executive, or any BSC member or associate member that may arise due to any loss or injury suffered by the above skater(s)
while he/she is travelling to or from, or participating in, any activity sanctioned by the BSC, Skate Canada Manitoba or Skate Canada.
I agree that the personal information contained on this form may be used by Skate Canada, Skate Canada - Manitoba Section,
and by their constituent skating clubs for the purposes of offering skating information and providing and administering skating
programs (including, but not limited to, instruction, testing, competitions, seminars and meetings).
